Convert Indesign source files to ePub & Mobi for sale

We require conversion of Indesign source to ePub and .mobi files for up to four titles. The titles are between 240 and 350 pages and the final files delivered must be "ready for sale" i.e. suitable for upload to aggregators and platforms such as Apple, Kobo, Amazon and Barnes & Noble.

We will initially commission a single title in order to assess the quality and process. If our requirements are fulfilled on this title then we will award a further project to convert the remaining three titles. We would therefore appreciate a quote on the initial title with rough estimates for subsequent titles should we proceed with them. Attached is an example title and we can provide a link to a DropBox folder with additional titles and the original Indesign source upon request.

Please include the cost of any extras (e.g. hyperlinks, revisions and refinements) in your quote. We have a preference for a fixed price quote and therefore this should include time for refinements and corrections to errors that have arisen during the conversion process. Any references or examples of similar conversion projects would be appreciated.
