Clean up a form code

I tried my hand a php, but complicated things. I now seem to have four forms on one page. I need them all to be merged into one form, so when user fills all out, one button is clicked and all the info in the form would be sent to the email in the php file.
I tried cleaning it up myself, by removing the tag from the end of each form; but it just messed up the layout completely. You see, each form has different css attached to it. and laid out on different parts of the one page.

It seems like a simple fix; but I cant figure it out. Help needed.

The php script I can pretty much do myself, once the form is fixed; so at present I just need someone to look at the form and fix it, so that the four forms become one without the layout being messed up.
