Need excel macro

Description of Business Development: Help in finding pharmacies
Extra notes: Attached is a list of pharmacies or drug stores in my area. Column D lists the street intersection where pharmacy is located. The intersection is written in the form "Street 1 + Street 2".
My patients tell me the pharmacy they want in the form "CVS at intersection of Street 1 & Street 2". I need to have a macro where I can enter name of the 2 streets and it should give me the street address located in column C.
Please note: there may be more than one pharmacy (with different names) located at same intersection. Also, some intersections are written as Stree 1+Street 2 and some are written as Street 2 + Street 1.

Ideally I would like the macro to work from the start button of the PC and not have to have excel document open.
