Customer Satisfaction Questionnaire

I had recently produced a basic questionnaire which was required for my business. I am not happy with the final product and I am looking for someone with a bit more skill than me to create a professional looking questionnaire form... ASAP.

I really need this as soon as possible as this was supposed to be in use today.

The questionnaire is for a Convenience store and I would like to find out if my customers are happy with the store, staff product quality and pricing etc etc etc.

Please let me know your quotes and time scale for this small project.
