Landlord & Tenant eviction

I am seeking assistance to formulate a statement in tenant eviction process for the up and coming court appearance as tenant refuses to vacate. I have couple pages of words and numerous photos of condition (poor) of inside and outside of my house. Tenant also refuses to all access to plumbing etc contractors to underake repairs.
so scope is to assist with my draft statement so most of the work is prepared just needs some polish!!
