Need a UK accountant to produce an accountants certificate

I need a UK based, preferebly chartered accountant to fill in an "accountants certificate" form for a mortgage application. I already have self assessment documents for the last 3 years but my mortgage lender has specifically requested that their own document is filled in. My current accountant is currently waiting on his membership renewal to come through so can't fill it in or sign as it would breach membership rules.

It's a very small form to be filled in and shouldn't be more than 2 hours work, but it's something that I need to be done urgently.
