Accountant to complete self-employed tax return 08/09

I'm looking for an accountant to complete my self-employed tax returns for the year 2008/9. Obviously, we're still in this tax year but with April fast approaching, I'd like to get someone signed up now.

I'm looking for a fixed-price quote on this. In your bid, please give details of your experience, qualifications etc. Also tell me what you'd need from me and when. A full schedule from signing up though to submitting the return to the tax man would be great.
