Visual Basic assistance

Looking for a Visual Basic expert to assist with coding for a document in Microsoft Word. The document is an engagement of business letter, sort of a contract, only 3 pages. I have attempted some coding myself and created an auto-fill form that inserts data into bookmarked parts of the document. I want the form to pop up when I open the macro-enabled document and the data in the fields to be inserted into different parts of the document. I am having difficulty getting the form to pop-up on opening and inserting the data from some of the fields into multiple parts of the document e.g. company name, invoice number etc... A simple task for someone versed in Visual Basic. Ability to demonstrate you have experience in Visual Basic coding and completed similar projects before is essential. 50% deposit and 50% on completion.
