Shipping labels

We have roughly about 700 contacts on Google Spreadsheet, which we need to merge into Google word doc (doesn't have to be good docs). The issue we have is the 'Primary Address' is too long for the Mail Merge. These shipping labels need to be printed on 'Avery Standard 5160', please use this merging setting.

We basically need this:
From: Our Company name
First Name Last Name
Their Company
Address
Country

We are NOT looking for someone to change our data, we are simply looking for someone to drop down the address line. This needs to be done ASAP, no time wasters, please :)
