Research to find an online membership system with questionnaire feature

I am trying to find a web-based membership system within which I can host content such as PDFs, Videos etc but also include multiple questionnaires.

I would like my members to be able to complete questionnaires within each module and for their answers to then be filtered into a sectioned document which can be edited and exported as a nicely designed PDF.

I know there are lots of membership systems around but it's the use of questionnaire-based modules which then export the data that I would like to explore.

I will need you to research and list out possible web-based systems that would support this requirement together with fees and features.

Good design and ease of use are important factors.
