Quantity Surveyor Forms for use with JCT Contracts - UK Projects

I need some QS Forms to be used mostly for Contract Administration, Payments, Cost Control, etc on
UK Construction Projects between £1M and £20M, under JCT Standard Building Contract 2016, JCT Design & Build Sub-Contract 2016 and other from JCT 2016 formats.
Price is just indicative and I am open to negotiation with UK based Quantity Surveyors with suitable selection / sets of forms.
They have to be in MS Word or MS Excel.
