Need Help to edit and redesign book

Description of consulting work: I have a book of just 21 pages. It need to be published on amazon as Kindle version and also I need the version to upload for Hardback/paper copy through Amazon
Because its just above 20 pages, the plan is to create a 5x7 type of booklet . At present the book is in word version.
Below are what is needed
- Design the front/back page, creating png, jpg, pdf format.. 2 or 3 designs is good
- Edit the book to create contents for copyright materials, acknowledgement, contents and the contents to fit 5x7 format.
-Review for typo or grammar errors
- Need support al through that both hard copy and kindle version is successfully uploaded on Amazon
