Microsoft Excel Risk assessment template

I am looking for someone that can create a fairly simple excel template that I can use to record all our company risk assessments. The template would need to be formatted so it could be printed (A4) and given to our site teams.

I would like some basic colour coding/conditional formating so that we can employ a traffic light system (Black for very high risk, red for high risk etc) so that the sheets are easy to follow on site.

I have uploaded a really good example for information purposes. We are not looking for a direct copy but it shows the sort of thing we are after. I have also uploaded the risk scoring guide that we would use to assess the risks.

I would like the sheet to be branded with our logo which I can supply in an eps or pdf format.

Please ask if any of that doesn't make sense.
