I need t's and c's

I am setting up a commercial IT and Telecoms consultancy and need some terms and conditions that the customer agrees when signing the contract.

As well as providing services i also rent equipment to the customer for a monthly fee - they never own the equipment unless a purchase price is agreed at the end of or during the contract. A contract is for a set period of time, normally 36 but up to 60 months and i require 90 days notice of cancellation after the initial period.

I am by no means a legal expert but have examples of competitor terms that could be used as a point of reference.

I am sure there are many other questions that you will need to be answered. I want to keep the agreement as simple as possible so as not to have pages and pages of terms that confuse the customer (but whilst still protecting the integrity of the contract, my business, and ensuring i can take action after non-payment etc)
