Format my Ebook for Smashwords

I've tried to format my Ebook to be included into smashwords premium catalogue but every time it bounces back. I'm getting very frustrated with it now.

I would like the person for the job to have an understanding of smashwords already, and what formatting is needed. The table of contents is formatted differently to the kindle format, with bookmarks.

When editing for smashwords, I did this over the top of the kindle format, so this may be part of the problem. Ideally it may need to be stripped of all formatting and started again.

Please send me proposals demonstrating your knowledge of smashwords and/or their formatting, or that you have read the requirements previously.
