Description of support work: Currently have 3 forms that have to be redesigned. The forms are PDF fillable, The basic structure will stay the same. Changes Implemented:

Make forms return address compatible for window envelopes #10 Window Envelope (return address only)

Make form slightly unique for 3 separate receivers (only 1 document must have this feature ) this allows us to
identify which credit bureaus this form applies to.

Make Form accommodate Short data entry (2 or 3 accounts on 1 page) this allows us to submit 1 page as we currently using 2 pages.

Make Form to accept inputted data in field removing lines once saved. This allows less ink being printed

Make 2 Forms accept more data (currently form accept up to 20 accounts) this allows less waste

Must have forms more efficient so expert input is valued :-)
