Fill in the fillable form fields on a 42 page pdf document

I have a pdf for a health questionnaire. The fields need to be made fillable. My previous freelancer has too much work and I need this completed this morning by 2pm.

You will need to understand English well enough to know which boxes are checkboxes and which checkboxes need to be mutually exclusive (only one box ticked) and which can have more than one box ticked.

The very last checkbox at the end of the form much be ticked by default.

I will be checking offers and allocating the job to someone at around 11:30am-12pm. The pdf is attached so you can see exactly what the job entails.

Please be sure you can complete this by 2pm even if I do not allocate the job until 12pm. That includes time for corrections so I hope you will be sending a file to me at about 1pm/1:30pm without too many mistakes and misunderstandings.

The text boxes should NOT be scrollable. i.e. when a client types in their data I want all of it to be visible within a field without scrolling within the box, so that I can print the form and see all of the data on the printed form.

Please make sure you are capable of achieving all of these requirements. Otherwise I will be seeking a refund. I have clients who are waiting for this work to be completed.
