I have a pdf for a health questionnaire. My previous freelancer has too much work and I need this completed this morning. I need it all ready by 10:30am.

Please do not apply for this job if you do not know you can achieve this in this timeframe. I will be checking offers and allocating the job to someone at around 8:30-9am. The pdf is attached so you can see exactly what the job entails. Please be sure you can complete this by 10:30am even if I do not allocate the job until 9am.

The boxes should not be scrollable. i.e. when a client types in their data I want all of it to be visible within a field without scrolling within the box, so that I can print the form and see all of the data on the printed form.

    Good afternoon! How do you do? I just wanted to clarify, you wanted to fill-in the form fields with your existing or preferred values, is that right?

    So that the PDF once completed will have all its fields filled-in or answered upon submission. Did I get it right?
