Word/Excel Template/Form creation - 1 page

In order to send Joining Instructions to our delegates, we currently edit a word document to change things like 'Venue' and 'Name' fields. We then save this as a PDF to send to our delegates.

SIMPLE VERSION
We would like a wizard type approach to this, so that certain field labels are protected, and the information within each field is easily changed.

I presume this would be a Word/Excel Template but am open to alternative suggestions.

COMPLEX VERSION
Not essential but nice if straightforward :
A calendar picker for the start and finish dates. Dropdown selectors (combo list preferred) for some fields with a selection of predefined options (such as Venue, Training Provider, Course Title). The selections would need to be editable, and we would need to be able to add new options to the lists ourselves.

PLEASE BID FOR THE SIMPLE VERSION AND STATE HOW MUCH EXTRA FOR THE COMPLEX VERISON.
