Word 2010 Form to be Created - URGENT

Hi,

I need a form to be created in Word 2010. I will provide an excel layout of what I need and a word letter head template to create on. I need the form to be created/designed on one side of A4 using the developer/form tools within Word 2010. There should be a function to reset/clear the form, locked for amending/editing the form etc.. and must be user friendly/professionally designed. This is an easy/quick job for a person who knows the Developer Function within Word 2010.

Please send me a proposal ASAP as I need to start sending this form out to prospective clients, so I will make a final decision on a proposal by three o'clock this afternoon (02/07/13).
