Web data capture and formatting

I need some relatively small data sets captured from four web pages:

1: Web page http://www.plasticsurgery.ie/find-a-surgeon/ There are 34 surgeons, and I need their key contact info captured into the attached spreadsheet.

2: The attached CYSPRAS Word document needs to be reformatted into a nice Excel sheet with appropriate column headings (name, address, e-mail, etc.)

3: The 45 people listed at http://www.spcpre.org.pt/site/component/comprofiler/userslist/S%C3%B3cios%20registados?Itemid=171 need to have their info transferred into a similar spreadsheet – Note that many doctors profiles contain only their name.

4: There are 11 clinics (some with a couple of doctors) listed at: http://www.suomenplastiikkakirurgiyhdistys.fi/?page=1003&lang=1 Once again, the names and contact data for these people need to go into an Excel spreadsheet. There should be one line for each doctor within each clinic (duplicate the clinic info as needed).

In each case the format (column headings) of the data capture spreadsheet will have to be slightly different (as slightly different info is given), but please use the "Ireland" Excel spreadsheet as a starting point in terms of layout and key fields.
