Timesheet and application form

hi,
I need some help to create a timesheet template where workers will need to enter their details , the date , start and end time with a space for them and the employer to sign every day. one timesheet should be for a week with our logo at the top, i also need to create an application form template with also our logo i started to create one to show you what i would like to achieve i have attached also the one i got inspired from. Also i need my workers to be able to sign their documents online so they won't have to print them out and the 48 hr out put letter that i have attached.
