Real Estate Cash Flow Sheet

We are a real estate investment company, needing to create "cash flow sheets" to show to our investors. Attached is a SAMPLE ONLY (3413 Capitol) of what the one page document should look like.

Ideally, we'd like a form where we can enter the values, have the calculations made automatically, and put them on the cash flow sheet. We'd also like to just upload the pictures to a form, and have those come out on the cash flow sheet too. It should look similar to the example, but design IS important.

We can fully explain each calculation as we move forward. There are about 10 of them.
