Powerpoint - Diagram needed to be inserted on a page for a course book

I'd like a diagram being inserted on to the format attached. This project has to be done in Powerpoint, exactly as shown. I've only attached 3 pages. The last page is where I'd like the diagram to go. This page will then be inserted into the full course.

See the instructions on the page. In a nutshell. I'm just wanting a circle or a square - large in the middle with four lines from each corner so delegates can write down their answers.

I've been trying for the last 30 minutes and can't figure out how to insert the image etc.

URGENT job, needed by mid day tomorrow Monday 16th 2018.

There is another job but I'd prefer to see how this job is conducted first before passing anymore work on.

MUST MUST HAVE KNOWLEDGE OF POWERPOIINT - this is not a slide, it's done in a course book A4 format...
