I need help making an interactive .pdf /writeable form from .pdf documents

I will have 10 .pdf files, each with 10-15 pages that will each require someone to turn them into interactive .pdf forms, adding text fields, checkboxes, etc. using Adobe Acrobat. This is detailed work that needs to be done fast! A sample .pdf is attached. Anywhere there is an existing checkbox, this needs to be "live" in the form as well as any place that someone might write text (e.g. where it says "ACTIVITY").
