I need five professional report templates designed.

I have designed five medico-legal templates for my company but I have run out of ideas (and time!). I now need five more developed to be used alongside the existing templates. I am willing to supply the five original templates to give you an idea of what information is required on them and the general look and feel of the documents in order to generate five additional versions.

All of these report templates will be adapted for use as virtual forms - therefore the design needs to be quite specific.
