I need editable forms creating in Microsoft Word from a design

Our client requires approximated 15 forms creating in Microsoft Word from designs that have been created by us.

We require the forms to have a simple footer, and a space on the first page of each document for the client to add their logo.

These forms need to be fully editable, and will consist of tables mostly.

There is a template to follow attached, the logo, the strap line as an image, the original word document we used to design the form, and also the form design for the professional version.

We would look to have one created first and make sure it is right, and then get the rest finished.

We require the file size to not be too large - so optimisation will help.
