I need a spreadsheet database turned into a a printable label format.

I am creating a mailing out and need to turn my data base into stickers that can be printed. I have an excel document with all the details but it will need to be manually imputed into a word document with a label template so we can print the labels from out printer.
Attached is a screen shot of how the excel spreadsheet is organised and is a template for the stickers. However, the sticker template is not a good one and I would prefer you to create a better one.
Urgent. This has to be completed by 09:30 UK time Tuesday 13th December.
