I need a Microsoft Word fillable form contract

I work with another photographer who gave me permission to copy his wedding contract form to make my own (I also do some jobs on my own). I made a new document with my watermark, header & footer. I need all of the rest of his form copied over, with my business name, Erica Lindsey Photography. I also need it to remain editable after completion in case I need to edit any of the details. So basically, just transferring from one to the other... I'm just not good with formatting the boxes & whatnot. I can send you a copy of his email stating that I can use his form, if necessary. I have attached my basic document as well as his form. Please give me an estimate of cost and when it can be completed. I'd like to have it done as soon as possible please. Also, my logo can't be used in any other projects as I have the rights to it. Thank you!
