Google Form with Archive for Group Faculty

I need a Google form created to allow for multiple teachers across the globe to provide recommended resources tying into specific themes for an upcoming online course. This form then needs to feed to a Google Spreadsheet where the data will remain in an archive and be input to the course over time.

I have a PDF that I will share that shows what resources are being requested. They include Audio, Video, Articles, Books, Scientific Papers, Creative Exercises. The resources shared need to be matched to specific themes and these themes appear at the top of each page in the PDF. For example, the theme for Month 1 is RELAX and there are 2 modules - Relax and Retune and Relax and Awaken. Ideally we will get suggestions from teachers for each resource type and for each module.

The spreadsheet archive will need to have clear headers with the ability to add additional notes on each teacher and each resource.
