Formulate a record sheet for a logistics company

I would like someone to formulate a sheet for recording the income and expenditure sheet to be completed for each individual trip for my logistics business. It’s just to produce a simple trip account record which is filled in for every individual trip which shows the revenue and expenses pertaining to that trip. I have all the details that need to go on the sheet and it will be filled in every time a vehicle goes on a trip. The sheet should be formulated such that when the relevant figures for the different items are input, it comes up with the totals and is able to do all the additions and substractions as required.
