Format headings and create a Table of Contents and Index in Word (2003)

Hello, I am currently working on the first draft of a non-fiction book in Word 2003. The book is currently 383 pages long (around 70k words) and I have put in headings, sub-headings and page numbers. What I need now is a Table of Contents and an Index for the book. I haven't used Word's standard Heading styles, so you would need to know how to use non-Word styles to form a ToC or change the heading and sub-headings.

I am hoping that the winning seller will be able to update the document's ToC and index in the final draft, if need be too, but just give a price for creating it first.

Please say when you can do this and the price. I have no idea how much this will cost!
