Excel Template Form

I'm looking for someone to create a form as an excel template. It is to
be used as a master template for creating work tickets for
manufacturing. I will enter data unique but common to each item that we
make. These will become templates for that item. I will then add the
order specific information (customer, quantity, etc.) for each release.
I have attached a hand draft of what I envision.

There will be a few very simple formulas but most of the template will be blank text fields. I imagine we'll go thru a couple of versions as I figure out what I really need.
