    I'd like to clarify the scope of the project.

    Does this simply include creating the database, or would you require any forms for data entry, and reports for extracting data? If so, would it be possible to have details of this?

    Also, would you require the database to be populated with existing data, for example from Excel Spreadsheets? I can migrate data from various formats.
