Create word doc form and type in names

Hi, I have had a bit of a guess at this job in terms of time so happy if you feel it will take longer etc. I have a PDF doc which I need converted to word (I have checked and you can cut and paste). I then need that formatted so that it looks like the word doc. And finally, I need a name entered on the form and the form saved. Then I need that repeated about 100 times as I have 100 names to put on the form. And lastly, I need it done by about 3pm if poss...! :)
