Create a Client Survey Form

New client here. I'd like assistance creating a very professional looking MS Word form to send to clients following an executive coaching engagement. I have already created the survey. Just need help making it look very professional (consistent with my firm graphics / themes). I want to have easy to use "voting buttons" for about 10 questions, and then about 10 questions with open text responses. Blue / Grey contemporary businesslike graphic style.
