Complete reformatting of a document

I require a the reformatting of an existing MS word document (attached) which will then be used as an MS WORD template so the document content and particularly the tables contained within the document MUST be easily editable so information can be added as required. Note the content is an incomplete exemplar.

I need this document back today by 2100 UK GMT. I have indicated a top cert rate so my expectation is this will be completed accurately and meet all exact requirements.
