I require the creation of an Excel template to digitize information from pages 20-21 of the attached PDF document. The PDF contains a certificate page and service record for benchmarking purposes. The template needs to capture all relevant data fields from these pages in an editable spreadsheet format. Some of the key information to extract includes the certificate number, issuance date, benchmark type, details of services/items certified, validity duration and terms of certification. The layout and formatting of the Excel sheets should closely resemble the visual design of the corresponding pages in the PDF. Conditional formatting and validation rules need to be added to the relevant fields for data integrity. Drop-down lists and calculations may also be included as needed. The objective is to have a digital file that can be populated easily for future certificates and records while retaining the official appearance of the original paper format. Candidates should have strong Excel skills along with experience in extracting and recreating information from paper to digital formats.
