Assistance with Portuguese Driving Licence application

I've just moved to Portugal and need some virtual assistance! Specifically, I need help with getting my UK licence switched to Portuguese. I understand I need to fill out a form, and potentially visit a doctor also. I also have a D1 (minibus) and C1 (lorry) licence which may slightly complicate things.

I'm in Braga, and I basically need a "how to" guide with the exact steps to take:

* List of necessary documents
* Location of doctors in Braga, with opening hours (pref evening or weekend)
* Location of the relevant authority where I need to take the documents (and opening hours)

Basically a "dummys guide" to getting this sorted (asap), along with some ad-hoc help along the journey!
