A appraisal template form to be formatted with drop down menus in microsoft word

I will be carrying out around 25 appraisals of my peers/colleagues and I want to develop a generic template form that I can use to make the writing up of the appraisal meeting easier. I will provide the basic text, and then I am looking for someone to expand on it (where necessary) and then to rephrase it in a style where it becomes generic. I also need drop down menus to be available for the different options, to enable easy population of the different choices.

I want someone who is skilled with microsoft word, and who has good written English.

These meetings start on this coming Tuesday 17th September, so this task needs to be completed before then. If you are unable to start work on this immediately, please do not make a bid.

A few lines example of what I am talking about is as follows:

I met you for your appraisal on INSERT DATE.Your have been in this role for INSERT NUMBER of years, and you carry out on average INSERT NUMBER assessments a year.
