I need 2 things taken from this site:

http://www.annuityline.co.uk/annuity/route3 (not our site)

1. We want the calculator quote box integrated - it doesn;t calculate anything - just a form over 2 sections that emails results to us.

Then the page refreshes for step 3 with thank details in the box.

2. The 'Request a call back' button - we want this on the page like how it is done on the site.

Just sends results via email and acknowledges sent on current page - could be done with a bootstrap alert.

There is also the request a call back button on our demo page near the bottom - this could just open the windows at the top of the page to save repeating it.
