Booking Calendar

I am in need of a booking calendar, where each day has a number of slots available for bookings. These are to be slots and not times ie - slot 1, slot 2, slot 3, slot 4 and then the user will click on one of these slots and be presented with a booking form. Once the form has been submitted that slot will be no longer available.

Requirements:

Calendar Based, Click a date and be presented with a page containing the available slots.

Booking form that will insert the data into a sql table and turn that slot status to booked. But the slot should be linked to an ID that when clicked on you can see the booking form in a disabled format.

Admin side of things ability to add additional slots per day and also to delete bookings.
