Book interior layout and formatting

Hello,

I am looking for someone who is familiar with Amazon’s Createspace publishing.
I need interior layout and formatting done for my self published poetry book, trim size 15.6 x 23.4 centimeters ready on a pdf so I can upload it directly to my createspace account

It is around 60 poems, some of them needs to be placed on two pages because of their length.
Also I need index made with the poem titles and page number on every page. Also book title on every page, and make sure everything looks neat and professional.
