Artwork for garment size/care label

I need artwork done for size/care labels for garment manufacturing. The size of each label is 3cm x 8cm and it includes company logo and care symbols, which I can provide. There will be one template in 2 colours, black and red (where specified) but 4 files because there are 4 sizes. Attached is my logo and care symbols. Here is how it should look:
