Experience Level: Expert

I want an app written in code that will work with PhoneGap. This job will also involve finding/creating a suitable database then connecting it to the app. All applicants must be prolific in JavaScript, HTML, CSS and PHP. A portfolio of your work would be extremely helpful. I already have a design from my graphic designer.
