Advanced PHP form interacting with SQL DB

I'm looking for a PHP form to connect with an SQL database. People have to fill the form with their Name Address Telephone country etc. Besides personal information there has to be the possibility for the customer to choose from a list of products (2 types of products with several variables). When 1 product is chosen they need the option of choosing an extra product (add extra field button). Each product has a value this value comes from the DB and has to be added tot the total. (based on the total a form will be accepted or declined) Last but not least there has to be a variable field to add customer products (only a name and product nr field, no values)
