Add shipping rates to wp website

I have a website built in wp storebox template by thematic the template has the functionality for add multiple shipping rates however doesn't seem to like it when I add more than two rates, I need this amended to allow for three shipping rates the functionality is in I just can't see what I have done wrong this is only a one hour job for someone who knows what they are doing.
