Add 31 new fields to database, add these fields to survey form, create pdf layout

We have a current system running on mysql web database which originates from a diary.

We want to add 31 new fields to the database. We then want out 'Survey form' to have these 31 fields displayed for editing.

We then want to hit a button to say create 'Operations Instructions' a pdf form in our specific layout. The form will show the details from the survey form.

Summary 31 fields on database, 31 fields added to survey form, create full pdf with fields in as attached.
