Activating questionnaire

I can do basic HTML and have done most of the web-site (in a "primitif" style which I think is needed) but questionnaires are beyond me. I need someone to gather the data automatically from my questionnaire (9 fields) and save it to a .csv database for use later by a mass-mailing programme. (Estimate: 2 hours' work) More work (e-commerce: more than I can manage) to follow.
