A5 NCR RECEIPT FORM

We need an A5 NCR compliment slip designed. We have logo, all we need is form designed.

Spec:
1) Logo, address, website address, telephone number top of page.
2) On main part of page we need, name, address, phone number, email address, media type and decryption fields.
3) One line of text down bottom of page.
4) Watermark of our logo in middle of page.
5) Two A5 Slips per A5 page.

We want this to be as good as possible, if you have experience with this type of work please provide examples, we want this work started over the next 2 days.
